Family Child Care Provider Inquiry Form

Complete this form if you are interested in becoming a Family Child Care Provider. To be a provider you must:

– Be 18 years of age or older
– Take 10 hours of health/Safety Training Online
– Be able to pass fingerprints/criminal background check
– Be able to provide care in the home where you live
– Attend a 2 hour Family Child Care Orientation Online
– Have a substitute provider
– Have a working email address
– Be able to register with NJCCIS.com
